The Patient
Lisa was 42, a senior executive at a tech company in Santa Monica, managing a demanding team in a high-stakes environment. She had been on Ambien for four years โ initially prescribed after a period of intense work stress that triggered severe insomnia. Despite the medication, she rarely felt rested, and had developed significant daytime anxiety that she managed with a combination of caffeine and willpower. Her physician had suggested reducing the Ambien, but every attempt resulted in rebound insomnia within two nights.
The TCM Assessment
Lisa's pattern was Heart and Kidney disharmony โ a classic insomnia presentation in Chinese medicine โ combined with significant Liver Qi stagnation from chronic work stress. Her tongue was red with a thin coating and a deep midline crack (indicating Heart Yin deficiency); her pulse was rapid, thin, and wiry. She reported waking at 2โ3am most nights, racing thoughts, palpitations when stressed, and a persistent sense of being "wired but tired."
The Treatment
Treatment focused on calming the Heart, nourishing Yin, and moving stagnant Liver Qi. Key points included HT7 (Spirit Gate โ the primary point for calming the mind), PC6, KD6 (to restore the Heart-Kidney communication), SP6 (to nourish Yin and Blood), and LV3 and GB34 to move Liver Qi stagnation. Janice also prescribed a modified Tian Wang Bu Xin Dan formula โ a classical Chinese herbal formula for Heart Yin deficiency and insomnia. Earseeds were placed at the Shen Men auricular point for Lisa to press throughout the day when anxiety spiked.
The Outcome
By week 2, Lisa was sleeping more deeply on the nights she took Ambien. By week 4, she began extending the nights between doses. At week 6, she had gone five consecutive nights without medication โ the longest stretch in four years. By the end of her initial 10-session course, she had stopped Ambien entirely, was sleeping 6.5โ7 hours most nights, and reported her daytime anxiety as "manageable for the first time in years." She now comes monthly for maintenance.
